Can you tell me one major functional defect you found during your current application testing?

Currently am working with a health care application and in this application I have tested a functionality where patient can book an appointment with the doctor. During my testing I observed an issue, Patient(Ram) booked an appointment with Doctor A(Raj) but the confirmation mail sent to Doctor B (Steve) with the appointment details.
